  • Stroke Specialty Programs

    Through a partnership approach among the person served, the family/support system, and the provider, Stroke Specialty Programs provide services that focus on prevention, on minimizing impairment, on reducing activity limitations, and on maximizing the participation and quality of life of persons who have sustained a stroke.

    These programs recognize the importance of ongoing medical management and the individuality, strengths, and needs of the persons served and their families/support systems. They provide ongoing access to information about services and resources available to enhance the lives of the persons served within their families/support systems, communities, and life roles.

    Stroke Specialty Programs assist the persons served and their families/support systems to accept responsibility for management of their own health, encourage their appropriate use of healthcare systems and services, and support their efforts to gain or maintain their health and improve their quality of life throughout their lifespan.
